As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

Additional Information
A referral (if provided) for physical therapy from your referring physician, ARNP, or PA. If you do not have a referral, no worries! As of September 1, 2019 a referral is no longer required to begin physical therapy
Some insurance coverage does require a referral prior to the initial visit, we save your time by verifying your eligibility and benefits, co-pay or your payment responsibility for u as a courtesy.
